Lasting Power of Attorney (LPA)

*OPG has extended the LPA Form 1 application fee waiver for Singapore Citizens to 31 March 2026*

Appointments are highly encouraged for LPA certification so that we can serve you in a timely manner

The LPA is a legal document which allows a someone to appoint one or more trusted persons (donee(s)) to make decisions and act on their behalf if he/she loses mental capacity one day.

​

A donee can be appointed to act in the two broad areas of personal welfare and property & affairs matters.

​

All LPA Forms need to be certified by an LPA certificate issuer. At Keystone Family Clinic, our doctors are accredited LPA certificate issuers. We will handle your needs sensitively and check that you understand the purpose and implications of making the LPA.

  • Helps you to make a personal, considered choice of a trusted decision maker, who will act in your best interests should you lose mental capacity one day

  • Alleviates the stress and difficulties faced by your loved ones who need to apply for a Deputyship order, if you lose mental capacity without having an LPA in place

 

How we can help you make a LPA 

  • You will need to be at least 21 years old, and have mental capacity

  • Apply for a LPA online using your Singpass

  • Make an appointment with our clinic

  • Bring your NRIC and your Singpass enabled mobile device to our clinic

  • If everything is in order, our doctor will electronically certify your LPA in the clinic

  • Only the donor needs to be present at the clinic. The donee(s) do not need to be present.

Advance Medical Directive (AMD)

An Advance Medical Directive (AMD) is a legal document that communicates to your doctors that you do not wish to have extraordinary life-sustaining treatment in the event you become terminally ill and unconscious. Extraordinary life-sustaining treatment refers to measures that will prolong your life but not lead to any meaningful recovery.

Making an AMD is a voluntary decision. It is entirely up to you whether you wish to make one. 

​

The AMD needs to be completed in the presence of two witnesses, one of which must be a doctor. Our doctors will explain the nature and implications of an AMD to you and ensure your understanding before we witness it.

How we can help you make a AMD

  • You will need to be at least 21 years old, and have mental capacity

  • The AMD form is available at our clinic, or you can download it here

  • Make an appointment with our clinic

  • Bring your NRIC to our clinic

  • Once completed, you will have to mail it to the Registrar Of AMD

  • The Registrar will send you an acknowledgement when the AMD has been registered
