A key pillar of Healthier SG is for everyone to have a lifelong relationship with a trusted family doctor. At Keystone Family Clinic, our family doctors will partner you as you embark on your Healthier SG journey by: 

• Working with you to develop a personalised health plan with practical and achievable goals

• Performing nationally recommended health screenings and vaccinations

• Providing timely and effective medical care for your acute and chronic health conditions

• Discussing your health condition(s) with you and addressing any health concerns you may have

 

Benefits of Healthier SG

• Upon successful enrolment, your will be eligible for a personalised Health Plan consultation, which will be free of charge

• After completing the initial consultation, you will receive 3,000 Healthpoints (worth $20) through the Healthy 365 app. Healthpoints can be exchanged for vouchers from participating merchants, including NTUC FairPrice vouchers and TransitLink credits.

• Singapore Citizens will enjoy free nationally recommended vaccinations and screenings 

• From early 2024, CHAS cardholders with chronic conditions can enjoy higher subsidies for their medications

• From early 2024, no cash co-payment will be required to use MediSave for chronic conditions

How do I join Healthier SG with Keystone Family Clinic?

Eligible Singapore Citizens and PRs aged 40 years and above will receive an SMS invitation from MOH to enrol for Healthier SG.

Simply select 'KEYSTONE FAMILY CLINIC' as your enrolled clinic using the HealthHub app! You can then book a consultation with our doctors for your first Healthier SG visit. Click here for a step-by-step guide.

Enrolment will be progressively extended to other age groups. Eligible residents will receive SMS invitations from MOH when they are eligible to enrol.
